A former nursing supervisor at the Glendale Nursing Home, who is facing neglect charges for allegedly ignoring an elderly resident who was suffering a cerebral hemorrhage, was previously cited in a similar incident by the state Health Department, department spokesman Robert Kenny said Monday.
On April 12, 1998, Sharon A. Caulfield, who was supervising the overnight shift at the facility and responsible for supervising and monitoring the care of residents, failed to document the assessment of a resident who had fallen at the home.
